Over at That Fresno Blog, there is a nice summary of a recently conducted downtown market analysis report. It has a pretty nice rundown of current housing developments and those in various planning stages (in a nice graphical form), along with some insight on the issues facing developers (mainly, a lack of funding now that the redevelopment monies are drying/have dried up).
The last graph is of particular note, especially that second bullet point: Businesses that cater to young professionals. I've always thought that a downtown gym (a nice one) would be a sure-fire money-maker. If Centerpoint can exist by serving the Tower District (essentially), it seems like downtown could totally support a gym of its own. Have it open early (and late) for all the workers who communte in each morning and out each evening.
It seems like such a natural progression (especially give the lack of competition) I can't believe George Brown hasn't figured it out yet.

The location depends on the intended clientele.
If you're looking to pull in downtown residents, somewhere in the Mural/Cultural Arts District could work. Something in one of the mixed-used developments (Fulton Village, etc.) would be great, though I haven't really seen a suitable space.
If the focus is on downtown workers, those commuting in and out every day, I'd put it somewhere near the galleria/Federal Courthouse area, someplace that looks nice/new/safe and is accessible to those on lunch break, etc.
I forgot about Centerpoint having the handball courts. I just know a lot of people who are members at Centerpoint, who I don't think would be members at gyms outside the area.
a gym would be a good addition. i knew people that used the ymca back in the day. one of my classmates in high school would go most mornings before school with his dad, who worked downtown. what part of downtown would it best fit in?
btw, i think one of the things that keeps centerpoint open isn't so much the tower district as it is the handball/racquetball courts. they have a lot, and they're usually full. centerpoint is the home of the fresno handball club.
